According to the police, there was a pit at ground level near the heat treatment plant, which contained 14 fire-extinguishing cylinders and other fire-extinguishing equipment.
However, the company had never sought permission from the authorities to construct the pit. The police say the pit was constructed illegally and, therefore, the standard operating procedures for its construction and maintenance were never followed. Both the DISH and police investigations found that the company had never applied for the required permission.

Maharashtra Police say they are also investigating how the pit was allowed to operate without permission, which authorities were responsible for conducting regular inspections of the plant, and how they failed to flag that the pit had been constructed illegally.
