Wind Warnings Were Never Given to Pilots in Miami Crash, Federal Report Says The pilots of an Amazon cargo jet had never worked together before the day of the crash, and were traveling at an abnormally high speed when they hit the runway, the report said.

An air traffic controller failed to warn the pilots of an Amazon jet about windy conditions when giving them permission to land at Miami International Airport last month, just before they overran the runway and crashed into two vehicles, according to a federal report about the incident published on Thursday. Five people died in the accident.

Federal regulations require controllers to tell pilots when a landing could be affected by strong tailwinds, which blow in the direction the aircraft is moving and can increase a jet’s speed over the ground, according to the report from the National Transportation Safety Board. The analysis did not conclude whether wind warnings might have changed the calculations of the plane’s captain, who pressed on toward the airport despite the co-pilot and the plane’s automated systems cautioning that the aircraft was approaching too fast.

When the airplane crossed over the start of the runway, it was moving at over 203 miles per hour and was still over a hundred feet in the air, according to the report. That approach was considered unsafe for landing.

The report offers insight into apparent communication breakdowns — between air traffic control and the pilots, and among the pilots themselves — in the moments leading up to the fatal crash on Sept. 6. The N.T.S.B. is not expected to identify a probable cause for the crash until it issues a final report, which may not be released for at least a year.

A spokesman for the National Air Traffic Controllers Association, a union representing controllers, declined to comment, citing an open investigation.

In a statement, 21 Air, the contractor operating the Amazon flight, said that the airline took safety seriously.
